Girls’ Basketball Tryout Form All girls trying out for basketball should fill out this form. This info will make the check-in process smoother on tryout day. Please contact the coach with additional questions.
Required 7th Grade Vaccinations All upcoming 7th graders are required to have proof of a 7th grade TDAP booster shot to start school. Please talk to you medical provider to get a print out of your child’s shot record to turn in when students return to school.
