Flu shots will be on November 16th at Lake Forest. Consent forms went home on Monday, November 2nd. Consent forms must be turned in no later than November 11th.

The Health Department will have a flu shot clinic for Virtual Students on November 10th, 12th, and 17th. The Health Department clinic will be from 2-4 each day.

In order to mitigate and slow the spread of COVID19, the following policies and
procedures have been put into place to support a safe reopening of school. The
staff of Lake Forest Middle School is committed to creating a safe instructional
environment while also providing an authentic middle school experience for our
students.

6th Grade Open House and Orientation is coming up!

All girls trying out for basketball should fill out this form. This info will make the check-in process smoother on tryout day. Please contact the coach with additional questions.

All upcoming 7th graders are required to have proof of a 7th grade TDAP booster shot to start school. Please talk to you medical provider to get a print out of your child’s shot record to turn in when students return to school.
